First Contact Physiotherapist

Within our Primary Care Network (PCN) we employ a first contact physiotherapist, Asim, who is able to take bookings without a GP referral. Asim is an independent prescriber and has access to request and scans or blood tests that he feels you would benefit from for your care or diagnosis. He can also provide steroid injections and refer onwards to secondary care physio services at the hospital if needed.

A few of the inclusion criteria for Asim include:

  • All soft tissue injuries, sprains, strains or sports injuries
  • Arthritis – any joints
  • Possible problems with muscles, ligaments, tendons or bone, e.g tennis elbow, carpal tunnel syndrome, ankle sprains
  • Spinal pain including lower back pain, mid-back pain and neck pain
  • Spinal-related pain in arms or legs, including nerve symptoms, eg pins and needles or numbness
  • Post-orthopaedic surgery
 

Care Enhancement Team

Our Primary Care Network (PCN) has set up and employed a team of clinical professionals to help prevent hospital admissions for our local residents and provide more services in their own home for them. The team currently consists of Nurses, Occupational Therapists, Social Prescribers, Care Co-Ordinators, Pharmacy Technicians and Clinical Pharmacists. They are able to see and support patients whose needs are not currently being met by other services. If you feel that any of your needs could be met by them with the below criteria, please do not hesitate to contact the practice and ask us to refer you onto the team. The team try to contact patients within a 48 working hour period.

What problems can the team support?

  • Non-compliance of medicines
  • Loneliness/ isolation
  • Housing problems impacting health
  • Financial problems impacting health
  • High intensity users of health services

Eligible patient will be:

  • Patients aged 18+ with complex multiple morbidities.
  • Patients not requiring GP care but may have social care needs.
  • Patients who are socially isolated and not engaging with services, which is having a negative impact on their wellbeing and/or mobility.
  • Patients with non-medical needs that require intervention in order to prevent escalation to providers in the wider health system

Physiotherapy

For any services that are not covered by our first contact physiotherapist, we are able to refer to a company called EKTRA, who hold a satellite clinic at the surgery once a week for patients to keep services local to the village for our patients.
